1. Completed Appraiser Application & Agreement
2. Copy of Your Current State Licenses & Certificate
3. W-9 Form
4. Errors and Omissions Insurance Certificate with a Minimum Coverage of $500,000
5. Two Reports Dated within the Last 30 Days Documenting Your Proficiency
